APTEUM SPINE + TSCN
Why We Joined TSCN
Patients with spine problems rarely need just one isolated visit. They may need imaging, therapy, pain management, another specialist, surgery, or follow-up—and every step has to connect to the next. We joined TSCN because we believe patients deserve connected, supported care pathways rather than being left to carry records, chase referrals, repeat their story, and figure out what happens next on their own. TSCN gives independent doctors a way to work together around the patient while preserving each doctor’s independent clinical judgment and the patient’s freedom to choose.
- Your next step should be clear.
- Your care should remain connected when more than one practice is involved.
- You should not have to become the messenger between your doctors.
- Referrals, records, results and follow-up should not disappear between offices.
- Your doctors remain independent, and your choices remain yours.
What TSCN Is
TSCN is a network of independent practices that coordinate care without giving up their independence. Practices remain independently owned. Doctors retain their clinical judgment. Patients retain their choice. TSCN supplies the connective infrastructure between practices.
Collective Access / Marketing
Help patients find participating independent practices.
Care Coordination
Connect intake, records, scheduling, referrals and follow-up across practices.
Fair Referral Engine
Match by clinical need, geography and availability; patient choice remains the patient's.
Care Loops
Track handoffs, results and next steps so care does not disappear between offices.
What TSCN Means for Patients
You should not have to coordinate your own care.
One person helps connect the dots when the path is unclear.
Less repeating your history and chasing records between offices.
Referrals and unresolved steps remain visible instead of simply disappearing.
Clearer next steps across participating practices.
More access to independent medical, surgical, imaging, therapy and behavioral-care options across New York and New Jersey.
Your choice, your voice, your doctor relationships and your doctors' clinical judgment remain yours.
What TSCN Means for Independent Doctors
Stay independent. Stop operating alone.
Shared infrastructure without selling or surrendering the practice.
Collective marketing that can generate demand no individual practice can build alone.
Care coordination connecting intake, records, scheduling and follow-up.
Fair referral routing based on need, geography and availability—not payment for referrals.
Care loops that track handoffs and next steps to closure.
Independent ownership, clinical autonomy, patient relationships and professional identity remain with each practice.
